BMH Physician Group announces the addition of Nancy J. Redfield, RN, MS, ARNP, BC to its Post-Acute Care Services.
Redfield will work with Eric Pofcher, MD to provide direct care for patients from the time of discharge from Brattleboro Memorial Hospital, or another acute care facility, to admission at a skilled nursing facility. Their duties include managing medical problems in post-acute medical episode recovery, stabilizing chronic conditions and supporting decisions for individualized end-of-life care.
An Advanced Registered Nurse Practitioner with 17 years’ experience, Redfield has provided evidence-based care with a focus on chronic illness, advanced disease, transitional care, oncology and palliative care in a variety of medical settings. She holds a master’s degree in nursing from UMASS-Amherst and is frequently invited to speak at care facilities throughout the region on the subjects of pain management, hospice and palliative care.
