The Chronic Disease, Chronic Pain and Diabetes Self- Management Program are all programs developed by the Division of Family and Community Medicine in the School of Medicine at Stanford University. They are all facilitated by two trained leaders, one or both of whom are peers that may also have the same condition as workshop participants.
